PUBLIC AGREEMENT (PUBLIC OFFER AGREEMENT)

on the provision of dental services at the UADenta clinic

1. General provisions

1.1. This Public Agreement (hereinafter – «Agreement») is an official offer of UADenta Dentistry (hereinafter – «Contractor»), addressed to an unlimited number of persons (hereinafter – «Patient») and posted on the Contractor's official website: www.uadenta.com.ua.

1.2. The terms of this Agreement are the same for all Patients, in accordance with Articles 633, 641 of the Civil Code of Ukraine.

1.3. The Agreement is concluded by the Patient's acceptance of the terms of this Agreement (acceptance).

1.4. The moment of conclusion of the Agreement is the moment of the Patient's confirmation actions (acceptance).

2. Terms and concepts

2.1. Acceptance - full and unconditional acceptance of the terms of this Agreement by starting to actually receive dental services, registering on the Contractor's website, ordering services through the website or by phone from the Contractor.

1.5. "Patient" - an individual who receives dental services at the Contractor's clinic.

2. Subject of the Agreement

2.1. The Contractor provides the Patient with dental services, the list and cost of which are determined in accordance with the price list posted on the Contractor's website.

1.6. The Contractor guarantees the quality of the services provided and the use of modern diagnostic and treatment methods.

2. Cost of services and payment procedure

2.1. The cost of services is determined in accordance with the Contractor's price list.

2.2. Payment is made by cash or non-cash payment.

1.7. The Contractor has the right to change the price list by publishing updated information on the website no less than 1 day before the changes come into force.

3. Rights and obligations of the parties

1.8. The Contractor undertakes to:

provide medical services of proper quality;
comply with medical standards and protocols;
inform the Patient about the state of his health, the list and cost of necessary services.

1.8. The Contractor has the right to refuse to provide services if the Patient violates the rules of conduct, is in a state of intoxication, or his actions threaten the safety of staff and other visitors.

1.9. The Patient undertakes to follow the doctor's recommendations, pay for services on time and provide reliable information about his health.

4. Warranty obligations

1.9. The Contractor provides guarantees for the performed dental services in accordance with the current legislation of Ukraine. The terms and conditions of warranty service are communicated to the Patient individually.

5. Responsibility of the Parties

1.10. The Contractor is responsible for the quality of the dental services provided.

1.10. The Patient is responsible for the reliability of information regarding his/her health and compliance with the doctor's recommendations.

5. Force Majeure

The Parties are exempt from liability for failure to fulfill the terms of the Agreement in cases of force majeure (force majeure).

6. Final provisions

This Agreement shall enter into force from the moment of its acceptance by the Patient and shall remain in effect until the parties fulfill their obligations.

All issues arising in the process of implementing this Agreement shall be resolved through negotiations between the parties.
